Offers a multi-year initiative by the Department of Health Care Services to improve the health outcomes of Medi-Cal Members. The program offers a comprehensive set of health care benefits and services. These services provided include:
- Enhanced Care Management (ECM)
- Community Supports
- Housing supports
- Short-term recovery supports
- Independent living supports
- Prerelease/in-reach care for people who are incarcerated
- Providing Access and Transforming Health (PATH)
- Population health management

Eligibility
Must be eligible members of either Anthem Blue Cross or Alameda Alliance. Qualifying people include:
People with significant behavioral health needs, including people with mental illness, serious emotional disturbance, or substance use disorder
Seniors and people living with disabilities
People experiencing homelessness who also have complex physical or behavioral health needs
People transitioning from jail or prison back to the community who also have complex physical or behavioral health needs
Children with complex medical conditions, such as cancer, epilepsy, or congenital heart disease
Children and youth in foster care
