Family Paid Leave (fpl)

Descriptions
Provides up to eight weeks of partial pay to employees who take time off from work to care for a seriously ill family member or to bond with a new child in the family (through birth, adoption, or foster care placement). Payments are about 60 to 70 percent of your weekly wages earned five to 18 months before your claim start date.
To receive benefits, client must:
- File a claim for PFL benefits using SDI Online or by mail (instructions for filing a PFL claim by mail).
- Have earned at least $300 in wages that are subject to State Disability Insurance deductions (look for "CASDI" on your paystubs) during the 12-month base period of your claim.
- Provide proof of relationship for child bonding claims (birth certificate or record, adoption paperwork, etc.).
- Have the physician/practitioner treating the care recipient complete the "Physician/Practitioner's Certification" for care claims.
