Ear Of The Lion Reconditioned Hearing Aids

Descriptions
Provides hearing aid devices for hard of hearing individuals in California and Nevada. Assigns an audiologist or hearing aid dispenser who will decide what type of hearing aid will be best for the patient. The audiologist/dispenser will make impressions of the patient's ear cavity and send it to a lab to be customized to the patient's need.
Eligibility
Individual must be low-income legal residents or citizens of the United States, who are diagnosed with a need for hearing aid devices but are not eligible for any government-assisted programs.
Annual Household income limit:
Family of 1: $22,540
Family of 2: $30,485
Family of 3: $38,430
Family of 4: $46,375
Family of 5: $54,300
*$8,500 is added per additional dependent

Documents
Copy of a hearing test taken within the past 6 months and medical clearance from a medical doctor.
- Verification of income.
- Completed application form.
Fees
One time fee of $150 per hearing aid loaned.
