Descriptions
Supports members with chronic or other serious health conditions that can improve with nutrition support.
Eligible members may receive:
-Phone appointment with a registered Dietitian Nutritionist
-Up to two meals per day (14 meals per week), or
-A weekly grocery box tailored to health needs
Services can be authorized for up to 12 weeks and may be reauthorized afterward if medically necessary.

Must be enrolled in Medi-Cal with CenCal Health Plan
