Descriptions
Supports members who are experiencing or at risk of homelessness and need a short-term residential setting to recover from an injury or illness.
The services in this program may include:
-Temporary housing with a bed and meals
-Limited or short-term assistance with grooming, dressing, showering, meal preparation, taking medication, and getting in/out of bed
-Coordination of transportation to post-discharge medical appointments
-Connection to ongoing services a member may require, including mental health and substance use disorder services
-Support in accessing benefits and housing resources
Recuperative Care cannot exceed six months per rolling 12-month period.
Eligibility
Must be enrolled in Medi-Cal with CenCal Health Plan
