Descriptions
Offers comprehensive care management to CenCal Health members with complex needs. Provides extra services to help members get the help they need to stay healthy, including coordinating the member's care from different doctors and service providers.
Serves the following Populations of Focus:
- Individuals Experiencing Homelessness
- Individuals At Risk for Avoidable Hospital or Emergency Department Utilization
- Individuals with Serious Mental Health and/or Substance Use Disorder
- Children and Youth Enrolled in California Childrens Services (CCS) or California Childrens Services / Whole Child Model (WCM) with Additional Needs Beyond the California Childrens Services Condition
- Children and Youth Involved in Child Welfare
Eligibility
Must be enrolled in Medi-Cal with CenCal Health Plan
