Offers comprehensive care management to CenCal Health members with complex needs. Provides extra services to help members get the help they need to stay healthy, including coordinating the member's care from different doctors and service providers.
Serves the following Populations of Focus:
- Individuals Experiencing Homelessness
- Individuals At Risk for Avoidable Hospital or Emergency Department Utilization
- Individuals with Serious Mental Health and/or Substance Use Disorder
- Individuals Transitioning from Incarceration
- Adults Living in the Community and At Risk for Long Term Care Institutionalization
- Adult Nursing Facility Residents Transitioning to the Community mes
Eligibility
Must be enrolled in Medi-Cal with CenCal Health Plan
