Services include:
Cultural Mediation, Culturally appropriate health education, Care Coordination, and building individuals or family capacity.
Eligibility
Must be a Medi-Cal recipient with Cencal Health.
Must be at risk of homelessness or experiencing homelessness.
Must be a resident of California.
