Descriptions
Provides children with behavioral health services. Children must be enrolled in one of the early education programs.
Services include:
- Assessment
- Child development
- Individual and family group rehabilitation and therapy
- Case management to connect children to services in the community
- Therapy plan
- Implementation of Therapy plan.
Eligibility
Children must be enrolled in a CommUnify early Education Program.
Application Process
By referral of Head Start, Early Head Start, and Home-Based programs of CommUnity
