Child Health And Disability Prevention Program (chdp)
Provided by SANTA BARBARA COUNTY PUBLIC HEALTH DEPARTMENT
Descriptions
Offers free periodic well-child examinations to low income children and youth in California. CHDP exam includes:
- Health and developmental history
- Complete physical exam
- Vision and hearing tests
- Immunizations
- Urine test, when indicated
If a condition is found, families are referred to private physicians and clinics for additional tests or treatment. Medi-Cal assistance is provided.
Eligibility
Children and youth are eligible if they live in California and are either:
-"Full Scope" Medi-Cal and Under 21
-Under 19, have no health insurance, and low income (through CHDP Gateway*)
