Descriptions
Provides one-on-one, personalized, and door-to-door transportation. For older adults to continue to receive medical appointments.
Eligibility
Must be 62 years of age or older and adults over 18 years old with mild disability.
Application Process
Call for intake application. Plan 10 days to application completion and 5 days for request of rides.
