Children's Palliative Care Services

Descriptions
Offers different services that provide help to children with complex medical conditions. Services provided include:
- END-OF-LIFE CARE: Provides sub-acute comfort care to life limiting ill or dying children and young adults. Professional team provides help with the needs of the child and family which address their different needs.
- TRANSITIONAL CARE: Offers help to children who are no longer benefitting from an acute care hospital yet are not able to be cared for at home.
- RESPITE SERVICES: Enables parents to take a break for a period of time from the home care of their child. Families can leave their child at GMCH where the child will receive care. They can also stay with their child at GMCH while their child receives respite care.
- BEREAVEMENT SERVICES: Provided to all families at GMCH who have lost a child or young adult for as long as the family needs the service.
Eligibility
Open to Children and young adults ages birth to 24 years. Upon review of clinical records.
